1 · Intestinal Displacement
Plant sterols/stanols are structural analogues of cholesterol. They incorporate into mixed micelles in the small intestinal lumen, physically displacing dietary and biliary cholesterol from absorption.
2 · ABCG5/G8 Efflux
The small amount of sterols that do enter enterocytes are pumped back into the gut lumen by ABCG5/ABCG8 transporters. This is why sitosterolemia (loss-of-function ABCG5/G8) causes massive sterol accumulation.
3 · Hepatic LDL-R Upregulation
Reduced cholesterol absorption → depleted hepatic cholesterol pool → SREBP-2 activation → increased LDL-receptor expression → enhanced LDL clearance from blood. This is the same downstream lever as statins, ezetimibe, and PCSK9 inhibitors — just upstream-different.
Sterols vs Stanols — the practical difference
Sterols (e.g., β-sitosterol, campesterol) have a double bond in the ring and are weakly absorbed (~0.5–2%) — meaning small amounts enter circulation, and these can be oxidized.
Stanols (e.g., sitostanol) are saturated (hydrogenated) versions — virtually unabsorbed (<0.04%) and cannot be oxidized. Equal LDL-lowering efficacy.
Practical implication: If the oxidized-phytosterol-atherogenicity concern bothers you, use stanols. Same LDL effect, no oxidation pathway. Brand: Benecol or any "plant stanol ester" product.

Caveat — Oxidized Phytosterol Binding
  • Oxidized phytosterols (oxyphytosterols) preferentially bind to Lp(a) particles

  • Oxidized phytosterols (oxyphytosterols) are atherogenic in animal models — relevance to humans using oxidized-sterol products is unclear. Eliminated by using stanols or capsule form.
